Mitigating Structural Valve Degeneration in Bovine Jugular Vein Valves
Bovine jugular vein (BJV) valves are widely used in congenital heart surgery but are limited in durability due to structural valve degeneration (SVD). Researchers from Columbia University Irving Medical Center have examined mechanisms of BJV prosthesis failure and tested BJV modification with poly-2-methyl-2-oxazoline (POZ), a polymer shown to inhibit protein and advanced glycation end-product (AGE) absorption. Clinical BJV valve explants demonstrated AGE, carboxymethyllysine (CML), and serum albumin (SA) infiltration, which was mitigated by POZ modification.
Key Takeaways:
- The research found that BJV prosthesis failure is caused by structural valve degeneration (SVD), which is characterized by AGE, CML, and SA infiltration.
- POZ modification was found to inhibit protein and AGE absorption, mitigating leaflet thickening, AGE and protein infiltration, and collagen disruption.
- The study used Von Kossa staining and immunohistochemistry (IHC) to analyze clinical BJV valve explants, and second-harmonic generation (SHG) imaging to analyze collagen structure.
- Poziid modification was shown to reduce leaflet thickness from 0.23 ± 0.07 mm to 0.03 ± 0.01 mm after 5-week in vitro glycation incubation.
- In vivo 28-day subcutaneous BJV leaflet implantations in juvenile rats demonstrated that POZ modification reduces AGE and CML infiltration.
Statistics:
- 13 clinical BJV valve explants underwent Von Kossa staining and IHC analysis.
- 5-week in vitro glycation incubation was used to assess the effect of POZ modification on BJV leaflet thickness.
- Leaflet thickness increased from 0.03 ± 0.01 mm to 0.23 ± 0.07 mm after incubation in the control group.
- POZ modification reduced leaflet thickness from 0.23 ± 0.07 mm to 0.03 ± 0.01 mm after incubation.
- 28-day subcutaneous BJV leaflet implantations were performed in juvenile rats to assess the effect of POZ modification in vivo.
