Standardized Medication Reconciliation Services Improve Clinical Outcomes in CHD Patients
Researchers from Hebei North University published a study in the Pakistan Journal of Medical Sciences, reporting on the efficacy of pharmacist-conducted medication reconciliation services for chronic diseases in elderly patients with coronary atherosclerotic heart disease (CHD).
